On the outside, you might look and feel fit. But what if your heart is telling a different story?

Recently, someone I know had a heart attack. They had no obvious symptoms beforehand and appeared healthy to those around them. They are now in critical condition. It was a wake up call not just for them, but for everyone in their circle, including me.

This blog is part of a four part series on heart health, prevention, and the steps we can take now to stay healthy long into the future. Today, we are starting with one of the most powerful tools in modern preventative health: the CPET test.

What Exactly Is the CPET Test

Unlike static assessments like blood pressure or cholesterol tests, CPET gives us real time data on how your body performs under progressive cardiovascular stress. You wear a mask to measure oxygen in and carbon dioxide out, sticky electrodes are attached to your chest which will measure your heart's electrical activity, also your heart rate, blood pressure, and breathing rate are monitored as you walk or run on a treadmill. You gradually increase the intensity until you reach your limit, revealing your true cardiovascular and respiratory capacity.

Why CPET Is More Powerful Than Static Tests

Unlike resting ECGs or blood pressure checks, which only offer a snapshot of your heart while at rest, CPET tells a much deeper story. Many heart conditions like ischemia, arrhythmias, or abnormal blood pressure responses only appear when the heart is placed under real stress. This test recreates those conditions in a safe, controlled setting. It shows us how well your heart, lungs, and muscles work together during progressive effort. We get real time data on your VO2 max, breathing efficiency, heart rate response, and energy system performance. It's a dynamic, gold standard approach and far more informative than anything a static test could offer. The CPET can help detect different cardiac and respiratory disorders and understand how they can affect the heart and lungs, including heart disease and heart failure.

  • VO2 max (your maximum oxygen uptake)

  • Anaerobic threshold (when your body shifts from using oxygen to other fuel sources)

  • Breathing efficiency ( how efficiently your heart and lungs deliver oxygen and remove carbon dioxide during exercise, poor efficiency means your body has to work harder to breath )

  • Heart rate and recovery response ( how your heart rate increases during effort and how quickly it returns to normal after exercise, a slower recovery can signal cardiovascular stress or reduced fitness )

Why This Test Matters

Many people walk around unaware that they are living with reduced heart or lung function. The CPET test helps identify this before it becomes a problem.

It can uncover:

  • Poor oxygen efficiency ( when your body struggles to use the oxygen you breath in, making activities feel harder than they should )

  • Cardiac limitations ( reduced heart function that affects how well your heart can pump blood during physical activity )

  • Pulmonary issues ( problems with your lungs that can restrict airflow or oxygen uptake, impacting endurance and breathing )

  • Ischemia (where blood flow to the heart muscle is reduced which could lead to a heart attack)

As mentioned most of these would not show up in a standard fitness test or basic health check.

What Can We Do With the Results

Once you have had your CPET test, you are not left in the dark. You get a full breakdown of how your cardiovascular system is performing and will be one of the most accurate ways to personalise your training and lifestyle to get real results and reduce your risk of heart disease.

In my mind, it’s worth knowing and being able to take the necessary actions rather than waiting for symptoms  or worse, having a heart attack and then having to recover. The test could show you have no heart risks at all. But even then, it will give us exact stats based on your personal results to begin to improve VO2 max and overall conditioning, which massively increases your chances of staying fit and healthy as you age.

Why Wait?

If you care about your fitness, your longevity and truly understanding your body, the CPET test is the gold standard for uncovering what is really going on inside your heart and lungs. It gives you the power to cover all the bases from preventing health risks to taking your training to an elite level.

There's no time like the present. It could be the most important decision you make for your long term health.
